Community Grants

photo of cute young girl with missing front teeth The Sonoma County Medical Association Alliance Foundation (SCMAAF) is a 501(c)3 non-profit organization that conducts fundraising activities to support its own community health programs as well as other nonprofit organizations that benefit the health and welfare of our community.   
The SCMA Alliance Foundation’s funding directive for the 2011-12 fiscal year is Promoting Mental Health to Vulnerable Populations (i.e., pregnant/post-partum women, children and the elderly).  Only non-profit 501(c)3 organizations that provide health services and/or health education are eligible for consideration. 
The application deadline is Monday, November 28, 2011. No applications received or postmarked after this date will be considered.  A PDF of the application may be emailed to SCMAAF Grants Review Chair, Lisa Sugarman, at lcsugarman@gmail.com. Grants typically range from $1,500 to $15,000. Applicants will be notified by December 20, 2011. Grant money will be issued by June 30, 2012, after the final accounting of the SCMAAF’s Annual Fundraiser, the Garden Tour. Organizations selected for consideration must agree to a reasonable collaboration with the SCMAAF to promote the Garden Tour.
Please provide the following:

  1. One page summary of proposed use of requested funding
  2. Narrative-no longer than two pages.  Submit information related to specific grant request.  Application will be evaluated on the following criteria:
    • Goals of the project
    • Timetable for achieving the goals
    • Number of people to be served by the program
    • Expected community impact of the program
    • Need for the program in the community
    • Matching funds or similar investment
  3. Please attach the following:
    • 501(c)3 certification
    • Mission statement
    • Budget for the previous fiscal year for the organization
    • Profit/Loss Statement for the previous fiscal year
    • List of current sources of income
    • Organizational chart with current list of Board of Directors
    • Grant Application form
  4. Additional relevant marketing material: (brochure, news articles, DVD, etc.)
